1/16/2023 EnchantaVaughn Approved Behavior: Todayâs clinical contact focused on building level of trust with the client through consistent eye contact, active listening, unconditional positive regard, and warm acceptance. Client appeared to grasp understanding of accepting responsibilities on his own and reducing symptoms of ADHD. Client displays an increased pattern of independence. Client was provided with learning opportunities for self-determination skills. The client was requested to identify areas in which she would like to improve (e.g. interpersonal relationships, grades, hobbies). Behavior: Client seemed to be enjoying the session, as evidenced by his interaction and attentiveness during selected activity. Client was observed paying attention and asking appropriate questions. Client presented an excellent attitude throughout the session. Client was supported for displaying active listening skills and attending to his therapy. Client engages in insightful conversations with QMHP and was observed answering questions gracefully. Client appeared to be in a good space and working hard, as reflected in his positive choices and full participation. Intervention: QMHP allowed client insight into several conflict resolution and problem-solving skills to utilize at home. QMHP encouraged the client to role play appropriate and inappropriate conflict resolution skills in efforts to highlight and demonstrate several appropriate options the client may utilize when he feels challenged or the need for impulsive disruptive actions. QMHP provided strategies such as positive reframing. QMHP assisted client with practicing reframing negative thoughts with more positive thoughts in efforts to reduce impulsive reactions to situations that usually trigger the client's negative expressions. Intervention: Client was able to reduce overactivity and become more attentive in session. QMHP discussed with the family one goal they believe they need to work on individually and as a family. QMHP had each member write down goals and discussed through brainstorming and creating a goals chart on how to achieve each one. QMHP prompted client to express how he believes his family should treat each other. QMHP encouraged client to think twice before reacting negatively or aggressively in situations that may not need that level of reaction to be solved. Response: Client smiled and was proud of himself. Client communicated he trusts his mom, dad, and grandmother. Client was acceptive of the love and support given to him by QMHP. Client demonstrates problem solving by staying calm and using his senses. Client acknowledges expectations and reviews goals and objectives with counselor and parent at the beginning of each session. Client displays good social behavior, as seen sharing and taking turns. Client implemented the positive reinforcement provided by his counselor. Client exhibited good manners. Response: Client focused on things he is good at and loves to do at home in order to stay motivated in the school/community setting and make something out of himself. Client did a good job ignoring distractions and maintaining appropriate behavior in the home. Client administered positive behavioral expectations, reducing ADHD. As the client has made more of his own decisions, he reported increased satisfaction and function. Client responded well to feedback, interacting well with his siblings. Client did not display any outbursts during the role play exercise. Client is making progress with his treatment.
